Accident Summary:

THE AIRCRAFT HAD LANDED AT NHA BE FOR FUEL. AT 1305 THE AIRCRAFT COMMANDER, CW2 MOORE, RECEIVED CLEARANCE FROM THE TOWER FOR HOVER DEPARTURE ON RUNWAY 29. AFTER MAKING THEIR HOVER CHECK, ALL INSTRUMENTS READING NORMAL, THE PILOT MADE THE TAKE-OFF. SHORTLY AFTER CLEARING THE RUNWAY THE AIRCRAFT COMMANDER INSTRUCTED THE PILOT TO MAKE A LEFT TURN OUT, HEADING GENERALLY WEST. ABOUT 150 TO 200 METERS OUT OVER THE NHA BE RIVER THE PILOT NOTICED HIS RPM DOWN TO APPROXIMATELY 5000. THE AIRCRAFT COMMANDER TOOK THE CONTROLS AND TOLD THE PILOT TO PUT THE EMERGENCY GOVERNOR SWITCH INTO THE MEERGENCY POSITION. NO RISE WAS NOTICED BY EITHER PILOT IN RPM AS A RESULT OF THIS SWITCH. THE AIRCRAFT COMMANDER INITIATED A FLARE TO SLOW DOWN HIS FORWARD AIRSPEED. SECONDS LATER AT A VERY SLOW FORWARD SPEED THE TAIL BOOM HIT THE WATER, THE AIRCRAFT ROCKED FORWARD AND BEGAN TO SETTLE INTO THE WATER. AS THE AIRCRAFT SETTLED IT TURNED ON ITS' RIGHT SIDE. AS THE AIRCRAFT COMMANDER WENT OUT THE LEFT DOOR THE PILOT PUSHED THE CYCLIC ALL THE WAY RIGHT TO ALLOW THE WATER TO STOP THE MAIN ROTOR BLADES. THE AIRCRAFT SANK IN ABOUT 20 TO 30 FEET OF WATER WITH THE ENTIRE CREW GETTING OUT SAFELY. NAVY PATROL BOATS PICKED THEM UP MINUTES LATER.\\
