Two days after coming back from a combat assault, it was noted by the crew chief that there was a four inch cut on top of the rotor blade. He found a two inch piece of radio antenna on the blade. Suspect a soldier walked into the rotor blade with a radio antenna.

Accident Summary:

On 6 September 1968, the 118th Assault Helicopter Company was on a predawn insertion near Hoc Mon RVN. The aircraft was filling slot number 9 in the formation. In the pick-up zone, 064 when loaded, picked up to a hover and made a hover check, followed by a take-off. Upon catching up with the formation, the aircraft entered a flare to slow the airspeed of 064 to that of the formation. When power was again applied, RPM bled off to a dangerous level. The aircraft commander reduced pitch so RPM could build again to a safe level. When the RPM had built up again, he attempted to pull on power again. Still none was available and the RPM bled off. Again pitch was reduced and an autorotation entered. A school type autorotation was made with a flare approximately 100 feet to slow the rate of descent. The aircraft touched down in a rice paddy with some ground speed. After it had slid one aircraft length, it struck an unseen rice dike which was hidden by the tall grass growing over the dike itself. The aircraft rolled onto its right side damaging the main rotor, drive train, tail boom and tail rotor. All occupants escaped injury and exited the aircraft without incident
